Surgical Instructions
Your First Visit
The following information will be needed at the time of your appointment:
Referral information/note from your dentist or doctor.
X-rays, if applicable (or verify that they have been sent to us).
A list of all medications (prescription, herbal, other) that you are currently taking.
Your physician(s) name(s) and telephone number(s).
Medical and dental insurance card along with the insurance referral, if applicable.
Completed Patient Information and Health History forms.

A parent or legal guardian must accompany patients who are under age 18 to all appointments. Patients who have limited English proficiency should bring a translator with them.
Planning & Preparation for Surgery
It is our commitment to provide compassionate care in the same way that we would care for a family member. We do our best to prepare patients for their surgery by providing detailed information on what to expect. Questions are encouraged and answered thoroughly. A comprehensive medical assessment is another important step in preparation. If extreme anxiety persists even after all questions have been answered, a patient may be offered a prescription for a relaxant medication that can be taken in the evening of the day prior to surgery and/or in the morning before leaving home for the office.
We want patients to be as comfortable as possible!
