Scheduling
Our office is open Monday through Thursday from 7:30am until 4:30pm and on Friday from 7:30am until 2:00pm. Consultation appointments are typically seen in the afternoon while surgical procedures are scheduled in the morning. We will make every effort to schedule your visit at a time that fits well with your schedule. If you have severe pain or an emergency situation, every attempt will be made to see you on the day that you call. We try our best to stay on schedule & to minimize the time that you spend waiting. Due to the fact that Dr. Worley provides surgical services, various unforeseen circumstances may lengthen the time that was allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience. |

We will ask that you arrive 10 to 15 minutes prior to your appointment (whether consultation or surgery) in order to finalize any last minute details. A preliminary evaluation (consultation) is necessary before most surgical procedures and is in the patient’s best interest. At this initial visit, personal registration information will need to be collected along with any medical and dental Insurance details. This appointment will consist of an assessment of the patient’s medical / health history, an examination, a discussion of findings (diagnosis) and finally any treatment recommendations. Relatively minor procedures such as a simple tooth extraction or a biopsy may in some cases be completed on the same day as the evaluation. However, a more complex medical history, a more involved treatment plan, or even an extremely full schedule will require that a second appointment be scheduled to provide treatment on another day.

Registration (EHR access)
At Mountain View Oral Surgery & Dental Implants, we strive to provide patients with an experience that is a “cut above”. One of the ways that we accomplish this is by providing a secure Electronic Health Record (EHR), provided by USHealthRecord.com. This clinical record is secure and completely confidential, operating behind a 100% HIPAA-compliant firewall. Best of all, this benefit is offered at no cost. The EHR provides access to your health information 24 hours a day, and can be used (should you wish) to store all of your medical and dental health information. You are able to view all clinical records of your time in this office; you can share access to these documents with other doctors by granting them access them to your record. If the doctor that you wish to add is not already in the USHealthRecord.com database, then you would need to add them to the database (a quick & no-cost process). If a doctor’s office does not wish to participate (add data to your record), then you can easily add your own information about your medical and dental visits.
In order to access your health record, follow these steps: |

Go to www.ushealthrecord.com and click on the button for “Patient Login”.- Enter your user name and password (provided to you by phone or email from our office). Please do not create a new username and password, but only use the ones given to you by our office!
- Click on My Chart tab at top of page & update edit your demographic & insurance information; to do this, find the Edit Info link at the top of the page beneath the “Basic Info” tab. To save changes at this page click the Submit button at bottom of page.
- Click on the Medical History tab at top of page and use Edit Profile to update your medical history.
- Look under the Communications tab and note that you may send a question using the Discussion tab. In order to be able to receive replies by email, be sure that the following email address is not routed to your Junk email folder: drworley@mvoralsurg.com. This is especially important for AOL users!
Troubleshooting EHR Access
If you have any difficulty accessing any of the Health Record webpages described above, follow these steps:
- Be certain that the “Underscore” button is used in your username, and not the “hyphen”.
- Check the security settings in your internet browser. Go to “Tools” menu, then “Internet Options”, then “Security” tab. Select the “Trusted Sites” button, then the “Sites” button. Uncheck the box beside “Require server verification…”; inside the box that says “Add this Web Site to the zone”, enter this address: www.ushealthrecord.com then click “Add”. Now repeat this for each of these addresses: www.rxdds.com, www.teamlinks.com, & www.ushrs.com . Now click “OK”, then “OK” a final time to leave the Internet Options menu.
- Pop-ups must be allowed for the program to function. No need to turn off your entire Pop-Up blocker. Simply allow Pop-Ups for this site. Go to “Tools”, then “Pop-up Blocker”. Allow Pop-Ups at the site.

The First Visit
The following information will be needed at the time of your appointment:

Referral information/note from your dentist or doctor

X-rays, if applicable (or verify that they have been sent to us)

A list of all medications (prescription, herbal, other) that you are currently taking

Your physician(s) name(s) and telephone number(s)

Medical and dental insurance card along with the insurance referral, if applicable.

A parent or legal guardian must accompany patients who are under age 18 to all appointments. Patients who have limited English proficiency should bring a translator with them.
Planning & Preparation for Surgery
It is our commitment to provide compassionate care in the same way that we would care for a family member. We do our best to prepare patients for their surgery by providing detailed information on what to expect. Questions are encouraged and answered thoroughly. A comprehensive medical assessment is another important step in preparation. If extreme anxiety persists even after all questions have been answered, a patient may be offered a prescription for a relaxant medication that can be taken in the evening of the day prior to surgery and/or in the morning before leaving home for the office. We want patients to be as comfortable as possible!
Anesthesia Options
There are three types of anesthesia available for most surgical procedures. Although the patient’s medical history and the complexity (and duration) of the surgery may dictate only one option in some circumstances, most often our patients have the option of choosing between one of the following anesthesia options. Please note that each option includes local anesthesia. Recall that local anesthesia (also called “Novacaine”) refers to the “numbing medication” that you may have received at your dentist’s office in the past. The difference between the options below has to do with the level of the patient’s awareness/consciousness.
Local Anesthesia alone – the patient is awake, though the relaxing distraction of music or DirecTV is available.
Local Anesthesia with Nitrous Oxide (laughing gas) – patients remain awake, though much more relaxed; a few will naturally fall asleep. Though not every patient actually laughs out loud, almost everyone becomes more relaxed. Nitrous Oxide (combined with oxygen) is delivered through a mask that fits over the nose. Of course, clear nasal passages are needed. A qualifier: the nitrous oxide mask can impede your surgeon’s access for some procedures that take place beneath the upper lip or in patients with very small mouths; in these cases it may not be an option.
- For either of these first two options, there are no strict dietary limitations. Feel free to enjoy a light meal before the appointment. You may want to avoid dairy products if nitrous oxide is planned.
Intravenous Sedation with Local Anesthesia –short-acting sedative medications are delivered which cause the patient to drift into a light sleep. The patient remains arousable (drifts in and out of full sleep) and can cooperate with positioning requests during the procedure, though in most cases they will have no memory of this afterwards. Since local anesthesia injections are still gently given, no pain is perceived so patients snooze comfortably & have no pain when surgery is completed.
General Anesthesia/Deep Sedation with Local Anesthesia –short-acting sedative medications are delivered which cause the patient to drift into a deeper level of sleep than is achieved with Sedation. The patient is fully asleep & not typically arousable until the surgery has been completed and the medication infusion has been stopped. Oral Surgical procedures that occur closer to the throat (removal of wisdom teeth for example) will typically require a General Anesthesia both for the patient’s safety (need for retraction vs. gag reflex) and comfort. Patients should awaken with no pain whatsoever, since local anesthesia is given while they are asleep.
For either Sedation or General Anesthesia:
- A mature adult must accompany the patient to the appointment and remain in our office during the procedure. After escorting the patient home, it is suggested that an adult remains with them throughout the day.
- Patients must not eat or drink for 6 to 8 hours before the appointment. Usual medications should be taken with a sip of water.
- State-of-the-art anesthesia monitoring equipment (blood pressure, EKG/heart tracing, and pulsoximetry) is used
- Comfortable, short-sleeve, loose fitting clothing should be worn so that we can easily place monitoring devices; if the weather is chilling, wear a jacket over your shirt that can be removed when you arrive.
To prepare for a surgical appointment, fill any prescriptions received during the evaluation prior to the surgical appointment.

Financial Information
Financial options are discussed during the initial visit. We are pleased to assist patients in choosing the best payment option for their individual needs. The patient (or guarantor) is ultimately responsible for all fees charged, regardless of insurance coverage.
Insurance
We will do our best to help you understand and make the most of your medical and dental insurance benefits. As a courtesy, we will attempt to verify your benefit coverage. However, remember that this verification is only an estimate of what your insurance company will pay. Any co-insurance and/or deductible owed will be due on the date of service. While we participate with many insurance companies, there are some that will not allow out of network coverage. As a courtesy, we file your insurance claim; however if the insurance company has not paid within 60 days from date of service, the balance is due in full from the patient / guarantor.
Non-Insured
For those patients that do not have insurance coverage for the planned treatment, we require payment in full on the day of service. We accept cash, checks (verified in the office), Visa, MasterCard and Discover.
